Dear Student Athlete:
On behalf of the Athletic Department and the Student Health Center, I would like to welcome you to Ashland University. As the Ashland University Team Physician, your health and safety while participating in intercollegiate athletics is my primary concern.
The National Collegiate Athletic Association (NCAA) requires all incoming freshman and transfer student athletes to have a sports physical prior to participation in intercollegiate athletics. This physical is specifically sports oriented and administered at the Ashland University Student Health Center.
Shortly after arriving at Ashland University, the athletic trainers will schedule your sports physical and advise you of the date and time. The cost of the physical is $30.00, payable by cash, check, credit card or Eagle Dollars at the time of your exam. This is a one-time cost, as sports physicals are only administered at the beginning of your athletic career at Ashland University.
Please print, complete, sign and date all forms listed below and return by mail to Ashland University Student Health Center or fax to (419)289-5209 prior to your arrival on campus.

- Insurance Questionnaire
- Sickle Cell - Please review the fact sheet. Complete the waiver form and return as directed. If you choose to have testing, the testing will be done at the time of your sports physical.
- Release of Information Form - allows health information to be shared between the Athletic Trainers and the Student Health Center.

In addition, all Ashland University students must complete the Medical History and Immunization Forms as part of your enrollment. These forms can be found on our website at https://www.ashland.edu/student-affairs/health-safety-title-ix/student-health-center/important-forms-and-information.
